UTM:RPM is meant to work in conjunction with UTM:Healthcare (see separate APP) which provides a connection to a patient’s health data and medical support team. As well, UTM:RPM integrates with the UTM messaging app which the hospital case managers use to receive alerts from the patients. Patients who have been discharged from the hospital with CHF, COPD, Diabetes, or Pneumonia have the UTM:RPM app on iPhone or iPad (iOS version 11+).
