The Health Passport (HSE) is a communication tool to support people to express their needs in a healthcare setting. This App includes sections on Critical Information, Personal Information, Communication, Decision Making, Medical History, Food and Drink, and Personal Care. There is an option to email the passport if required. The Health Passport (HSE) will support healthcare staff to get to know the needs of the passport holder, enabling them to consider reasonable adjustments before any assessment, examination or treatment of a person.
