Is a technological infrastructure to prevent disability. The approach is to monitor the Intrinsic Capacity (physical, mental and psychological) from home in older than 65 to provide the elderly tailored interventions. The platform involves all the relevant actors (patients, caregivers, healthcare professionals –primary care and specialists-doctors and nurses).
